Key Responsibilities
Manage patient populations with chronic illness and/or multiple co-morbidities
Coordinate with multiple PCPs
Serve as the central link to an identified population of patients
Required Qualifications
Graduation from an approved, accredited school of Nursing
Washington Registered Nurse License upon hire
2 years of work experience in an RN role
Experience managing complex patients in an ambulatory setting
Work experience with medications and IV solutions
Preferred Qualifications
Bachelor's Degree in Nursing
