As a key member of our rehab team, you’ll deliver evidence-based therapy, participate in interdisciplinary care planning, and help residents achieve their personal goals.

Key Responsibilities:

• Evaluate residents and create individualized treatment plans
• Provide skilled physical therapy to improve strength, balance, and mobility
• Document all treatments and progress in compliance with regulatory standards
• Collaborate with nursing, OT, and SLP teams to ensure coordinated care
• Educate residents, caregivers, and staff on safe movement and rehab strategies

Qualifications:

• Active Virginia PT license (or eligibility to obtain)
• Experience in a SNF or rehab setting preferred
• Strong communication, organizational, and documentation skills
• Compassionate, patient-centered approach to therapy
• Commitment to professional integrity and clinical excellence
