Key Responsibilities
Conduct in-home assessments including medical history and physical exams
Perform evidence-based practice screenings
Identify diagnoses for care management
Formulate lists of medical conditions
Communicate findings to inform member’s PCP
Identify and intervene in urgent situations
Educate members on disease processes and medication
Required Qualifications
Active NP license or ability to obtain by start date
Active ANCC or AANP national certification in relevant areas
Ability to complete physical requirements of the job
Access to reliable transportation for travel
Preferred Qualifications
Additional licensure in other geographic areas
1+ years of clinical experience
Experience in Gerontology, Cardiology, Internal medicine, or Endocrinology
Home Health care or home visit experience
