The Care Management Social Worker plays a crucial role in restoring patients' health and facilitating their transition from hospital to home. This position involves collaborating with healthcare teams, coordinating care, and providing advocacy and support to patients and their families. Ideal candidates will possess a Master's degree in social work and have experience in acute settings.
Key Responsibilities
Collaborates with Care Transition team for high-risk patients and ensures timely follow-up appointments
Coordinates patient care activities with healthcare team and community partners
Intervenes in suspected abuse/neglect cases and high-risk situations
Identifies and mobilizes patient and family strengths for optimal resource use
Participates in discharge planning activities and addresses psychosocial complexities
Maintains patient records and performs psychosocial assessments
Provides patient advocacy and assists with financial program applications
Required Qualifications
Master's degree in social work/welfare from an accredited school
Two years of Social work or Case Management experience in an acute setting or protective services
Preferred Qualifications
Active certification in Case Management (ACM or CCMC)
Current and valid license as a Clinical Social Worker issued by the State of California Board of Behavior Science Examiners
