Job Description

Join The Jewish Board as a Senior Case Manager and make a meaningful impact on the lives of adults with serious mental illness. Support residents in achieving independence through personalized recovery goals and a collaborative approach while advocating for their needs within the community.

Key Responsibilities

  • Carries a caseload and practices person-centered service planning
  • Evaluates and assesses independent living skills
  • Advocates on behalf of residents with other service providers
  • Independently completes documentation of all resident interactions
  • Participates in intake, assessments, discharge, and follow-up conferences

Required Qualifications

  • Bachelor of Arts (BA) or Bachelor of Science (BS) degree
  • Experience working with individuals living with serious mental illness, substance use disorders, or trauma

Preferred Qualifications

  • At least 1-year experience in Human Services
  • Familiarity with Microsoft Office suite
