Job Description

Join The Jewish Board as a Case Manager to empower adults with serious mental illness towards independent living. You'll provide essential support, advocate for residents, and collaborate with community services, all while fostering a compassionate, culturally competent environment.

Key Responsibilities

  • Carries a caseload and practices person-centered service planning
  • Advocates on behalf of residents with other service providers
  • Participates in intake, assessments, discharge, and follow-up conferences
  • Completes regular documentation of all resident interactions
  • Facilitates resident support groups and meetings

Required Qualifications

  • Bachelor of Arts (BA) or Bachelor of Science (BS) degree
  • At least 1-year experience in Human Services preferred
  • In-depth understanding of mental illness, substance use disorders, and trauma
  • Experience working with individuals living with serious mental illness, substance use disorders, or trauma

Preferred Qualifications

  • Familiarity with Microsoft Office suite
  • Valid driver's license and willingness to drive agency van preferred
