Job Description
Join The Jewish Board as a Case Manager to empower adults with serious mental illness towards independent living. You'll provide essential support, advocate for residents, and collaborate with community services, all while fostering a compassionate, culturally competent environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Advocates on behalf of residents with other service providers
Participates in intake, assessments, discharge, and follow-up conferences
Completes regular documentation of all resident interactions
Facilitates resident support groups and meetings
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
In-depth understanding of mental illness, substance use disorders, and trauma
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van preferred
