Key Responsibilities
Provides direct patient care as per the Nurse Practice Act
Implements the plan of care initiated by the Registered Nurse
Documents care accurately and timely
Educates patients and families on care plans
Coordinates home health services with the clinical team
Follows infection control standards and safety guidelines
Participates in personal and professional growth
Required Qualifications
High school diploma or equivalent
Graduate of an accredited practical nurse or vocational nursing program
Current unrestricted license in Texas as a Licensed Vocational Nurse
Current CPR certification
2+ years clinical experience in community home health or medical/surgical setting
Knowledge of current nursing practice and regulations regarding home care
Valid Texas Driver's License and personal auto insurance
Preferred Qualifications
Experience working with electronic medical record applications
Basic computer skills including Microsoft Word and Outlook
