Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams for care coordination
Provide direct patient care including case management and counseling
Maintain timely patient-centered documentation
Participate in administrative and development activities
Support the development of hospital programming
Required Qualifications
Master's Degree in Social Work, Counseling, or Marriage and Family Therapy
LMSW, LPC-A, LMFT-A, or CADC
Previous experience in a healthcare setting
Valid Driver's License
Computer proficient
Excellent communication skills
Preferred Qualifications
Experience with interdisciplinary teams
