Job Description

Key Responsibilities
Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function
Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes
Provide written evaluation and plan of care defining occupational therapy needs, goals, progress and expected outcomes
Deliver occupational therapy services in compliance with prescribed physician orders
Maintain plan of care through timely documentation and modification of goals and treatment as needed
Collaborate and educate patients and caretakers on home exercise plans for discharge
Coordinate services with healthcare team members
Document effectively to avoid technical or clinical denials
Supervise other personnel (OTA, HHA) as applicable
Complete OASIS assessments where allowed
Required Qualifications
Degree in Occupational Therapy from an accredited university/program
Current, unrestricted state license as an Occupational Therapist in the state of practice
Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
Preferred Qualifications

Proven relationship-builder
Experience in Home Health Therapy and completion of OASIS documentation
