As an IRIS Consultant at Molina Healthcare, you will build meaningful relationships with individuals in the Wisconsin IRIS Program, helping them navigate their long-term care goals. This remote position requires compassion and resourcefulness as you assist participants in achieving their desired outcomes through personalized plans and community connections.
Key Responsibilities
Meet with IRIS participants at least four times a year
Provide orientation to new participants about their rights and responsibilities
Assist participants in identifying personal outcomes and ensuring they are met
Document planning activities and progress in the IRIS data system
Research community resources to support participants
Maintain confidentiality and HIPAA compliance
Work collaboratively with other staff and attend team meetings
Required Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related field
1+ year of direct experience in social services
Knowledge of Long-Term Care programs
Excellent problem-solving and critical thinking skills
Valid driver's license with good driving record
Preferred Qualifications
Experience with individuals with intellectual or physical disabilities
Familiarity with the principles of self-determination
