Physical Therapist (PT) – PACE Program

MedAdventures is looking for a compassionate and experienced Physical Therapist (PT) to join a PACE (Program of All-Inclusive Care for the Elderly) team in Loveland, CO . This is a rewarding opportunity to support older adults through individualized care, helping them maintain mobility, independence, and quality of life within their community.

You’ll work closely with an interdisciplinary team to deliver therapy in both the clinic and home environments, tailoring care to meet participants' functional goals.

• Evaluate and treat PACE participants to improve strength, balance, and mobility
• Develop and adjust personalized care plans based on individual needs
• Collaborate with physicians, nurses, and the interdisciplinary team
• Educate participants and caregivers on home exercise programs and safety
• Document progress clearly and accurately in accordance with program standards

• Active Colorado PT license (or eligibility to obtain)
• Previous experience with geriatric populations or PACE programs preferred
• Ability to work collaboratively with a full care team
• Strong communication and clinical reasoning skills
• Passion for delivering patient-centered care in both home and clinic settings
