Key Responsibilities
Deliver skilled nursing services in-home including wound care, medication administration, diabetes care, post-surgical care, and injections
Work 1:1 with patients, building trust and rapport
Collaborate closely with care team and patient's physician
Maintain clear, accurate documentation and comply with DOH standards
Required Qualifications
Licensed Practical Nurse (LPN) licensed in Pennsylvania
CPR certification
Ability to travel locally with own vehicle
Skilled communicator with a patient-centered approach
Preferred Qualifications
Experience in home health or long-term care
