Key Responsibilities
Provide comprehensive biopsychosocial/spiritual assessment and diagnosis
Deliver clinical counseling, assessment, case management, crisis intervention, and psycho-education
Collaborate with interdisciplinary team to develop and implement care plans
Respect patient self-determination and adhere to NASW code of ethics
Required Qualifications
Master's Degree in Social Work from a CSWE Accredited School
Oregon Clinical Social Worker License upon hire
2 years relevant social work experience
Preferred Qualifications
2 years clinical work experience in Hospice or Palliative Care
3 years social work experience
2 years working independently with vulnerable populations in a field environment
