Job Description

Key Responsibilities

  • See patients in various clinics, homes, and through telemedicine
  • Collaborate with WellMed physicians and staff
  • Provide coordinated care to Medicare-aged patients
  • Educate providers on coding and quality protocols
  • Ensure proper documentation and preventative screenings
  • Track performance using organizational metrics

Required Qualifications

  • Certification through AANP, ANCC, or NCCPA
  • Current Licensure through Texas Board of Nurse Examiners or Texas State Medical Board
  • Valid DEA license or ability to obtain one
  • 1+ year of clinical experience
  • Comfortable managing a Medicare-aged patient population
  • Ability to commute around greater San Antonio
  • Availability to work designated hours of 8am-5pm

Preferred Qualifications

  • Familiarity with primary care and chronic condition codes
  • Bilingual (English/Spanish) fluency
  • Understanding of managed care environment
