Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. In this role, you will act as a case planner, providing essential support and resources to clients to help them lead safe, independent lives.
Key Responsibilities
Routine community contacts on behalf of social worker
Locates and utilizes community resources for clients
Evaluates living conditions of clients through home visits
Interprets agency functions to clients
Provides structure and guidance in independent living skills
Runs groups under supervision of social worker
Evaluates clients for individualized treatment programs
Identifies and facilitates client referral to service providers
Conducts individual/group counseling
Performs community outreach and establishes linkages
Provides case management to clients
Maintains necessary documentation and paperwork
Required Qualifications
BA Degree
Experience in community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
