Care Manager, LTSS (RN) - Field travel in Marquette and Columbia County, WI

Key Responsibilities

• Completes face-to-face comprehensive assessments of members
• Facilitates comprehensive waiver enrollment processes
• Develops and implements case management plans
• Conducts ongoing monitoring of care plans
• Promotes integration of services for members
• Assesses for medical necessity and authorizes waiver services
• Conducts face-to-face or home visits as required
• Facilitates interdisciplinary care team meetings
• Uses motivational interviewing to educate and support members
• Identifies barriers to care and provides assistance
• Conducts medication reconciliation when needed
• Requires 50-75% travel
