Key Responsibilities
Provide clinical coverage for the primary APC during periods of PTO or absences
Manage members through acute medical situations or chronic condition exacerbations
Conduct comprehensive and routine assessments
Effectively manage acute and chronic medical and behavioral conditions
Ensure accurate and complete documentation
Communicate and collaborate with the interdisciplinary care team
Required Qualifications
Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
Active and unrestricted license in the state of residence or ability to obtain by start date
Active Nurse Practitioner certification through a national board
Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
Ability to gain a collaborative practice agreement if applicable
Ability to complete physical requirements including moving a 30-pound bag
Driver’s license and access to reliable transportation
3+ years of clinical experience in practice (long-term care setting preferred)
Preferred Qualifications
5+ years of clinical experience in practice (long-term care setting preferred)
Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
Understanding of Geriatrics and Chronic Illness
Proficient computer skills including documenting medical information
