Job Description

Join UnitedHealthcare as a Field Based Care Coordinator, where you will manage complex care for older adults with medical and behavioral health needs. Your role will involve coordinating care plans, engaging with members, and collaborating with healthcare teams to enhance quality of care and improve health outcomes.

Key Responsibilities

  • Serve as primary care manager for high medical risks/needs members
  • Engage members face-to-face and/or telephonically to complete comprehensive needs assessments
  • Develop and implement individualized, person-centered care plans
  • Partner and collaborate with internal care team, providers, and community resources
  • Provide referrals and linkages as appropriate

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse in Indiana
  • Master’s degree in social work
  • Bachelor’s degree in social work, psychology, special education, or counseling with at least 1 year of case management experience
  • Licensed practical nurse (LPN) with a minimum of 3 years of clinical experience with older adults
  • 1+ years of experience with local behavioral health providers and community support organizations
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • 1+ years of experience in long term support services or working with older adults
  • Resident of state of Indiana
  • Ability to travel 75% of the time within assigned territory
  • Access to reliable transportation & valid US driver's license

Preferred Qualifications

  • Demonstrated experience/additional training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care
  • Experience working in team-based care
  • Experience in serving individuals with co-occurring disorders
  • Background in Managed Care
  • Bilingual in Spanish or other language specific to market populations
