Join CenterWell Senior Primary Care as a Primary Care Physician and lead a dedicated team in delivering high-quality, coordinated healthcare for seniors. This role focuses on direct patient care and improving outcomes through a collaborative, integrated care model.
Key Responsibilities
Evaluates and treats center patients according to standards of care
Monitors care using available data and chart reviews
Coordinates patient services including specialty referrals and home health care
Participates in daily huddles with the care team
Completes medical record documentation in a timely manner
Follows policy and protocol defined by Clinical Leadership
Participates in growth opportunities for new or existing services
Assures compliance with licensing and accrediting bodies
Spends 100% of time on direct patient care
Required Qualifications
Current and unrestricted medical license or willing to obtain
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
High level of skill with interpersonal relationships
Engaged in integrated team-based care model
Willingness to learn and adapt to value-based care
Superior patient/customer service
Basic computer skills, including email and EMR
Preferred Qualifications
Board Certification or eligible in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Medicare Provider Number
2-5 years of directly applicable experience
Experience managing Medicare Advantage panel of patients
Knowledge of Medicare guidelines and coverage
Bilingual
Knowledge of HEDIS quality indicators
