Physical Therapist – Long Term Care/SNF - Travel Position

Location: Portland, OR

Job Description:

We are seeking a compassionate and skilled Physical Therapist to join a skilled nursing and long-term care facility in Portland, OR, on a travel contract. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide therapy services aimed at improving mobility, independence, and overall quality of life. The ideal candidate is patient-focused, adaptable, and experienced in geriatric rehabilitation.

Key Responsibilities:

Conduct thorough patient evaluations to assess functional abilities, mobility, and rehabilitation needs.
Develop and implement personalized treatment plans in coordination with physicians and care teams.
Provide interventions including therapeutic exercise, gait training, balance activities, and mobility support.
Educate residents, families, and caregivers on therapy goals, safety practices, and use of assistive equipment.
Document evaluations, progress notes, and treatment outcomes in compliance with facility and regulatory standards.
Collaborate with interdisciplinary staff to ensure coordinated care and optimal patient outcomes.
Promote patient independence, safety, and quality of life through evidence-based therapy practices.

Qualifications:

Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program.
Current and valid Physical Therapist license in Oregon (or compact license).
Minimum of 1 year of recent experience in long-term care or skilled nursing facilities.
Current certification in Basic Life Support.
COVID-19 vaccine and recent tuberculosis test.
Strong communication, clinical, and organizational skills.
Compassionate, patient-focused approach with experience supporting geriatric populations.
