Key Responsibilities
Provide professional, comprehensive, patient-centric care management services for at-risk patients
Conduct assessment and planning, coordination of care, resource utilization management, and discharge planning
Document interventions and ensure regulatory compliance
Advocate for patients and ensure appropriate healthcare resource usage
Required Qualifications
Associate's Degree - Nursing degree/diploma upon hire
Washington Registered Nurse License upon hire
2 years - Acute care experience in a Medical Surgical or Inpatient setting
Preferred Qualifications
Bachelor's Degree - Nursing or higher within 3 years of hire
National Certification in area of specialty upon hire
1 year - experience in care management or utilization review in any setting or successful completion of TIPS program or Case Management Orientation Program
