Job Description
Join The Jewish Board as a Case Associate and make a significant impact on the lives of New Yorkers. In this role, you'll work as a case planner to develop action plans and connect clients with essential resources, while providing case management and support.
Key Responsibilities
Function as a case planner supplying appropriate interventions for client care
Develop action plans utilizing existing and new community resources
Conduct routine community contacts and home visits
Evaluate living conditions and assess independent living skills of clients
Run groups under social worker supervision
Perform individual/group counseling
Provide case management and outreach to establish linkages with other groups
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual in Haitian-Creole
