Key Responsibilities
Provide basic care and medical treatment to individuals served
Complete health assessments and provide input for nursing care plans
Follow up on physician orders and medical/dental appointments
Monitor, administer, and record medications
Review medication error forms and recommend procedural changes
Conduct quality assurance checks on health care records
Maintain confidentiality and respect human rights
Coordinate services with external agencies
Administer and track testing logs for staff and individuals
Provide training to staff and individuals on various topics
Assist with household activities and transport individuals as needed
Participate in the on-call system for emergency coverage
Required Qualifications
Associate Degree in Nursing or related field
Graduate of an accredited LPN program
One year of experience working with population served in a nursing capacity
Current state LPN License
CPR/FA certification
HIV/AIDS certification
BBP certification
ACT training
Current driver's license
Car registration and auto insurance
