Join MJHS as an RN Case Manager to provide essential hospice and palliative care services, supporting patients and families through challenging times. In this field-based role, you'll assess patient needs and implement care plans, making a meaningful impact in their lives.
Key Responsibilities
Visit patients to perform physical and psychosocial assessments
Electronically document visit occurrences
Develop and implement nursing plans of care
Make recommendations for care based on patients' needs
Contact the patient's physician for care plan changes
Supervise, train and evaluate home health aides and homemakers
Required Qualifications
Associates/Diploma from an accredited school of Nursing
New York State RN licensure
Valid NYS driver's license
Preferred Qualifications
BSN
At least two years experience in medical-surgical, community health, and/or oncology nursing
Proficiency with Word and Excel
Experience working within an interdisciplinary team environment
Effective communication, listening and teaching skills
