$2,500 Sign-on Bonus!

 

Position Summary

In this role, you will be responsible for the management of the community’s wound program. This includes
assessment, prevention, evaluation, implementation and education on wound prevention and care. A deep
understanding of wound management principles, resident care, and ability to collaborate with the healthcare
teams to promote positive outcomes is essential.

 

Essential Functions, Key Duties, & Responsibilities:

• Conduct comprehensive assessments and maintain accurate and up-to-date documentation of wound assessments,
treatments, and patient responses.
• Conduct wound rounds with rounding practitioner.
• Identify factors affecting wound healing and develop person centered care plans.
• Complete treatments per physician orders.
• Educate residents, families and associates on wound care management and prevention strategies.
• Provide hands on training and support to nursing staff regarding best practices in wound care and prevention.
• Work closely with physicians and other healthcare professionals to ensure coordination of care.
• Collaborates with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically
met.
• Participate in quality improvement initiatives related to wound care, including tracking infection rates, pain
management, and healing outcomes.
• Participates in the survey process when inspection surveys arise.
• Other duties as assigned.

 

Education: A Nursing degree from an accredited college or university.

Licenses/Certifications: Current unrestricted license as a Registered Nurse (RN) preferred, LPN with extensive wound management
experience.

 

• Strong assessment and critical thinking skills.
• Certification in wound care is preferred.
• CPR certification is preferred.
