Key Responsibilities
Establish trusting relationships with patients and their families
Coach patients in effective management of chronic health conditions
Convey the purposes and services of programs to users
Assist patients in understanding care plans and instructions
Document activities and service plans effectively
Advocate for patients and act as a liaison between patients and community service agencies
Help patients utilize resources and schedule appointments

Required Qualifications
Must be 18 years or older
Must have a high school diploma or GED
Must be a U.S. Citizen or Permanent Resident/Green Card holder
Must be able to pass a national criminal background check
