Job Description

Join the Beacon Brook Center for Health & Rehabilitation as an MDS Director (RN) and lead the MDS program to ensure compliance and quality resident care. Your expertise will directly impact the lives of our residents while you grow your career in a supportive and collaborative environment.

Key Responsibilities

  • Manage the Minimum Data Set (MDS) assessments
  • Ensure compliance and accuracy in assessments
  • Determine Patient Driven Payment Method (PDPM)
  • Monitor Case Mix Index (CMI) scores
  • Train and mentor MDS Coordinators
  • Coordinate comprehensive resident assessments and care planning
  • Collaborate with interdisciplinary teams
  • Track and analyze assessment data for compliance
  • Drive a culture of continuous improvement in nursing care

Required Qualifications

  • Valid state RN license preferred
  • Direct care in a long-term care setting and/or MDS Coordinator experience required
  • Proficient in state and federal regulations governing the MDS and billing process
  • Knowledge of Electronic Medical Record (EMR), PDP, MDS 3.0, Medicaid and Medicare requirements
  • Strong organizational skills and analytical capabilities
  • Excellent written and verbal communication and interpersonal abilities
  • Ability to work effectively in a multidisciplinary team environment

Preferred Qualifications

  • Advanced degree or certification preferred
  • Interest in the nursing needs of the aged and the chronically ill
