Job Description
Join The Jewish Board as a Case Manager to empower adults with serious mental illness. Utilize a person-centered approach to support residents in achieving independence and recovery within the community.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Advocates on behalf of residents with service providers
Completes regular documentation of resident interactions
Participates in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
