Join Optum as a Field RN specializing in hospice and palliative care, where you'll provide essential in-home and telephonic nursing services to patients with life-limiting illnesses. Your role will be pivotal in enhancing patient outcomes and ensuring quality end-of-life care.
Key Responsibilities
Conducts telephonic and in-home patient assessments
Conducts urgent/acute visits to keep patients out of the hospital
Provides ongoing assessments of patient needs
Implements and revises individualized care plans
Performs nursing procedures including wound care and injections
Educates patients and families on disease processes and end-of-life care
Collaborates with interdisciplinary team members
Documents patient care in electronic health records
Required Qualifications
Registered Nurse with a current license
Current BLS certification or must obtain within 30 days
3+ years of experience in a physician’s office, clinical, hospice or hospital setting
Demonstrate experience of excellent nursing skills
Proficient computer skills including Microsoft Word, Excel, Access and Outlook
Reliable transportation for daily travel
Valid driver’s license
Excellent verbal and written skills
Ability to interact productively with individuals and multidisciplinary teams
Excellent organizational and prioritization skills
Preferred Qualifications
Hospice and Palliative Care Nursing certification
2+ years of hospice experience
Experience with home visits
Experience related to advanced care planning
Experience with community-based nursing
Knowledge of palliative and hospice medicine
Bilingual (English/Spanish) proficiency
