Director of Care Solutions (Discharge Planner/Case Management)

Are you an RN or LVN with a passion for guiding patients through successful care transitions? We’re looking for a dynamic leader who combines clinical expertise with strong communication and team leadership skills to make a real difference in patients' lives.

What You’ll Do:

• Drive effective care coordination, case management, and discharge planning

• Lead and inspire an interdisciplinary team to achieve exceptional outcomes

• Oversee care reviews, family education, and transition support

• Monitor patients for changes in condition and adjust care plans accordingly

• Communicate with case managers, caregivers, and staff to ensure seamless care

• Supervise the Care Navigator and participate in QAPI initiatives

• Align your work with company goals and contribute to our Balanced Scorecard success

What You Bring:

• A valid TX or compact RN or LVN license that is good standing with the State

• Solid knowledge of SNF regulations, reimbursement, and case management

• Previous case management and/or discharge planning experience in the skilled nursing industry is required. 

• A collaborative spirit and a results-driven approach
