Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Provide direct patient care including case management, care coordination, and crisis intervention
Refer patients to appropriate levels of care and available resources
Maintain timely patient-centered documentation
Required Qualifications
Master’s degree in social work, Counseling, Marriage and Family Therapy or related human service field
Previous experience in a healthcare setting, preferably with mental health and/or chemical dependency population
Current Connecticut LMSW, LPC-A, LMFT-A, or CADC
Excellent communication skills
Valid Driver's License
