Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning
Evaluates and assesses independent living skills
Advocates on behalf of residents with service providers
Completes regular documentation of all resident interactions
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
