Work Hours: Monday - Friday, 8:00 AM - 5:00 PM, 36 hours per week (3 shifts of 12 hours each)
Pay: $2,120 weekly ($630 taxable, $1,490 non-taxable)

Job Summary:
The Registered Nurse will provide direct patient care services to hospice patients and supportive care to their families. Responsibilities include managing an interdisciplinary team of nurses, nursing assistants, and support staff to ensure high-quality hospice care, as well as case managing all assigned patient clinical records. The nurse will perform initial and ongoing assessments, develop and implement the Hospice Plan of Care, monitor patient conditions, maintain accurate medical records, and provide education and support to patients and families.

Duties:

• Provide direct patient care and supportive services to patients and families
• Conduct initial and ongoing patient assessments
• Develop and implement Hospice Plan of Care in collaboration with the interdisciplinary team
• Report changes in patient condition to physicians
• Maintain accurate and up-to-date medical records
• Monitor scheduling and performance of staff
• Participate in quality assessment and performance improvement programs
• Educate patients, families, and caregivers on care needs, medications, and hospice philosophy
• Promote company core values and comply with safety standards

Required Skills & Experience:

• Minimum of one year of RN experience in acute care, oncology, long-term care, hospice, or home health
• Knowledge of hospice patient care and family needs
• Strong patient assessment skills
• Understanding of roles of all disciplines in hospice services
• Good communication and documentation skills
• Knowledge of regulatory guidelines and managed care principles
• Ability to operate nursing assessment tools and basic office equipment
• Transportation for work is required

Education & Licenses:

• Current RN license in the state of employment
• BLS (American Heart Association)
• Valid driver’s license and automobile insurance
