Key Responsibilities
Deliver physical therapy services in accordance with physician's plan of care
Assess and evaluate patients' therapeutic, rehabilitative, and functional status
Develop and direct individualized therapy programs
Evaluate home environments and recommend modifications
Train patients and caregivers in therapeutic appliance use
Provide instruction and support to families/caregivers
Supervise and guide Physical Therapy Assistants (PTAs)
Monitor and report patients' responses to treatment
Prepare clinical and progress summaries
Required Qualifications
Active Physical Therapy license in Indiana and/or Ohio
Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
CPR certification
Recent TB test
Valid driver's license
Reliable transportation
Auto insurance
Preferred Qualifications
2+ years of physical therapy or rehab experience
Community/home health experience
Strong communication skills
