You'll lead a team of RN Case Managers and ensure high-quality in-home care in accordance with agency policies.
Key Responsibilities
Direct case management of assigned clients
Plan, implement, and evaluate care provided
Collaborate with regional director and other personnel to improve services
Serve as a local on-site clinical resource
Participate in interviewing and training of clinical personnel
Review clinical documentation for accuracy
Participate in performance improvement initiatives
Required Qualifications
Graduate of a state approved school of professional registered nursing
Current, unrestricted RN license in the state(s) of practice
Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
Current CPR certification
Preferred Qualifications
BSN preferred
One (1) year of supervisory and/or case management experience preferred
