Job Description
Join UnitedHealthcare as a Clinical Combination Care Manager to simplify health care for communities. You'll assess and advocate for members' needs while managing their care plans, ensuring they receive the best possible support.
Key Responsibilities
Assess, plan and implement individualized care strategies
Perform the NYS UAS Assessment in the member’s home
Identify and initiate referrals for social service programs
Manage the care plan throughout the continuum of care
Communicate health-related information to stakeholders
Advocate for patients and families
Required Qualifications
Current, unrestricted RN license for the state of New York
2+ years of relevant clinical work experience
1+ years of experience in community case management for individuals with complex needs
Experience in long-term care, home health, hospice, public health or assisted living
Proficiency with MS Word, Excel and Outlook
New York state issued ID or ability to obtain one prior to hire
Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings
Preferred Qualifications
Behavioral health or clinical degree
Experience with electronic charting
Experience with arranging community resources
Field-based work experience
Background in managing populations with complex medical or behavioral needs
Proficient in use of UASNY
