The RN Care Manager provides comprehensive case management to Medicare and Medicaid managed care members by developing and monitoring individualized care plans. Collaborating closely with primary care providers, caregivers, and multidisciplinary teams, the role addresses medical, social, psychological, physical, and spiritual needs to promote patient independence. This position requires a New York State RN license, nursing experience, and a commitment to high-quality healthcare delivery within managed care settings.
