Job Description
Join Centene as a Care Manager and play a critical role in transforming healthcare for our 28 million members. You will develop and manage care plans for members facing mental and behavioral health challenges, ensuring they receive high-quality, cost-effective care while collaborating with families and healthcare providers.
Key Responsibilities
Evaluate member needs via phone or in-home visits and recommend care plans
Perform telephonic, digital, and home site visits to assess needs
Develop ongoing care plans for members with high acuity
Coordinate between members, families, and care provider teams
Monitor care plans and member outcomes, providing recommendations as needed
Facilitate care coordination with providers and specialists
Document member information and ensure compliance with regulations
Provide education to members and families on healthcare options
Offer feedback to improve care quality and delivery
Required Qualifications
Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing
2 – 4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Bilingual candidates highly preferred
Experience in pediatric care (for Sunshine Health in Florida)
