Job Description
Join HighPoint Hospice as a PRN RN Case Manager and make a difference in end-of-life care. Develop trusting relationships with patients and families while ensuring quality care through continuous evaluation and coordination of care plans.
Key Responsibilities
Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient
Conduct initial nursing evaluations for eligibility of hospice services within 48 hours of referral
Identify and reassess patient/family needs at least every 15 days
Initiate and coordinate the plan of care while maintaining accurate clinical notes
Document problems, goals, interventions, and patient/family responses
Collaborate with patient/family and healthcare team members in providing care
Required Qualifications
Current RN licensure in the state of practice
One year of clinical experience
Current CPR Certification
Current driver's license
Valid vehicle insurance
Access to a dependable vehicle or public transportation
