Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the community by providing essential case management and client care. Collaborate with social workers to develop action plans and connect clients with vital resources, all while enjoying comprehensive benefits and a supportive work environment.
Key Responsibilities
Function as a case planner and ensure client care
Develop action plans to identify and utilize existing resources
Routine community contacts on behalf of social worker
Evaluate living conditions through home visits
Interpret agency function to clients
Provide structure and guidance in independent living skills
Evaluate clients for treatment programs
Facilitate client referrals to service providers
Perform individual/group counseling
Conduct home visits to assess family and child safety
Provide case management to clients
Perform community outreach and establish linkages
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
