Staffing & Ratios

• RN:Patient Ratios :
  • Days: 1:4
  • Nights: 1:4–5
• Nurse Aides :
  • Days: 1:8
  • Nights: 1:12
• Charge Nurse :
  • Days: No patient assignment
  • Nights: 1–2 patients
• Monitor Techs :
  • Present. They handle monitoring, measuring, documenting
  • RNs not required to be telemetry experts

Scheduling & Requirements

• Shift Times : 6:45 AM – 6:45 PM (12-hour shifts)
• Self-Scheduling : Allowed
• Block Scheduling : Allowed (more common on nights)
• Weekend Requirement :
  • Every other weekend or 4 shifts/month
  • Flexible
• Holiday Requirement :
  • Rotating schedule – every other of the 6 major holidays:
    • New Year’s, July 4th, Thanksgiving, Christmas, Memorial Day, Labor Day
• Call/Standby : None
• Floating : Possible – travelers rotate with perm staff

Patient Population

• Common Diagnoses/Procedures :
  • Pre/Post cardiac procedures (stents, pacemakers, ICDs, PCIs)
  • Heart caths (femoral & radial)
  • Cardioversions
  • Cardiothoracic (CABG, thoracotomy – from PACU)
  • Vascular procedures
  • NSTEMI
  • Trachs (with home vents, BiPAP, CPAP)
  • Chest tubes
• Pacer Wires : Pulled in ICU (not this unit)

Required Certifications

• BLS
• ACLS
• NIHSS : Preferred

Required Skills/Experience

• Strong cardiac background
• Experience with:
  • Stents, pacemakers, ICDs (pre/post)
  • CABG (pre/post)
  • Vascular patients (pre/post)
• Comfortable with titrating cardiac drips

Drips

• Common Drips :
  • Cardizem
  • Amiodarone
  • Dopamine (set rate)
  • Dobutamine (set rate)
  • Nitro
  • Integrilin
• Titration :
  • Most are titrated except for Dopamine and Dobutamine

RT/Respiratory Support

• CPAP
• BiPAP
• Home vents (experience not required )

Orientation

• Unit-specific Orientation : 2–3 shifts
