Job Description

Key Responsibilities

  • Perform comprehensive assessments and document findings
  • Ensure that all diagnoses are ICD10 coded accurately
  • Address and document quality elements
  • Conduct initial medication reviews
  • Complete all mandatory educational and training requirements
  • Travel between care sites as necessary
  • Communicate medical concerns with patients and team members

Required Qualifications

  • Certified Nurse Practitioner or Physician Assistant through a national board
  • Active and unrestricted APC license in Connecticut
  • Current active DEA licensure or ability to obtain post-hire
  • Access to reliable transportation for client visits
  • Availability to work 10 hours per week
  • Ability to gain a collaborative practice agreement if applicable

Preferred Qualifications

  • 1+ years of hands-on post grad experience within Long Term Care
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including documentation with medical records
  • Ability to develop and maintain positive customer relationships
  • Adaptability to change
