Job Description

Key Responsibilities
Perform comprehensive assessments and document findings
Ensure that all diagnoses are ICD10 coded accurately
Address and document quality elements
Conduct initial medication reviews
Complete all mandatory educational and training requirements
Travel between care sites as necessary
Communicate medical concerns with patients and team members
Required Qualifications
Certified Nurse Practitioner or Physician Assistant through a national board
Active and unrestricted APC license in Connecticut
Current active DEA licensure or ability to obtain post-hire
Access to reliable transportation for client visits
Availability to work 10 hours per week
Ability to gain a collaborative practice agreement if applicable
Preferred Qualifications
1+ years of hands-on post grad experience within Long Term Care
Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Proficient computer skills including documentation with medical records
Ability to develop and maintain positive customer relationships
Adaptability to change
