Job Description
Join The Jewish Board as a Case Associate, where you'll work directly with clients to develop tailored action plans for their care. In this role, you'll provide essential support, conduct home visits, and connect clients with community resources, making a meaningful impact in their lives.
Key Responsibilities
Develop action plans for client care
Conduct routine community contacts
Evaluate living conditions through home visits
Run groups under supervision of the social worker
Individualize client treatment programs
Perform individual/group counseling
Provide case management
Establish linkages with community resources
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
