Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You'll function as a case planner, developing action plans and providing essential support to clients while enjoying excellent benefits and a collaborative work environment.
Key Responsibilities
Function as a case planner responsible for client care
Develop action plans for clients within their community
Conduct routine community contacts and home visits
Evaluate living conditions of clients
Provide individual/group counseling
Identify and facilitate referrals to treatment or service providers
Perform community outreach and establish linkages with organizations
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
