Key Responsibilities
Conduct in-home assessments including medical history and physical exams
Perform evidence-based practice screenings
Identify diagnoses for care management
Formulate lists of medical conditions
Communicate assessment findings to PCPs
Identify and intervene in urgent situations
Educate members on disease processes and compliance
Required Qualifications
Active NP license or ability to obtain by start date
Active national NP or PA certification or ability to obtain by start date
Ability to complete physical requirements of the job
Reliable transportation for home visit assessments
Preferred Qualifications
May be requested to obtain additional licensure in other geographic areas
1+ years of clinical experience in their highest level of education
Experience in gerontology, cardiology, internal medicine, or endocrinology
Home health care or home visit experience
Ability to travel throughout the state, up to 5% of the time
Fluency in additional languages
