Key Responsibilities
Lead and support the development and implementation of a Care Management program
Provide comprehensive care management to patients with chronic conditions
Support the Transitions of Care (TOC) team as needed
Conduct assessments, develop care plans, and monitor patient progress
Engage patients and families in self-management education and support
Participate in interdisciplinary team meetings
Ensure compliance with billing and documentation standards
Track and report on patient outcomes and program impact
Required Qualifications
Washington Registered Nurse License upon hire
Preferred Qualifications
Bachelor's Degree in Nursing
Certified Case Manager upon hire
2 years of Clinical experience
Experience in utilization review/case management
