Job Description

Key Responsibilities
Evaluate and treat center patients according to care standards
Monitor patient care using available data and chart reviews
Coordinate patient services including referrals and home health care
Participate in care team daily huddles
Assist in setting a cooperative practice tone
Complete medical record documentation timely
Follow policy and protocol defined by Clinical Leadership
Meet with RMD about quality of care and policies
Participate in growth opportunities for services
Participate in local primary care on-call program
Ensure compliance with licensing and accrediting bodies
Focus 100% on direct patient care
Required Qualifications
Current and unrestricted medical license
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
High level of interpersonal relationships and communications
Engagement in integrated team-based care
Willingness to learn in a value-based care setting
Superior patient/customer service skills
Basic computer skills, including email and EMR
Preferred Qualifications
Board Certified or eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
Active and unrestricted DEA license
Medicare Provider Number
2-5 years relevant experience
Experience managing Medicare Advantage panel
Knowledge of Medicare guidelines
Bilingual
Knowledge of HEDIS quality indicators
