Assess patient needs, develop care plans, and monitor progress toward health goals. Collaborate with physicians, interdisciplinary teams, and family members to ensure continuity of care. Educate patients and caregivers on treatment plans, medications, and self-care strategies. Accurately document patient assessments, care plans, and outcomes. Facilitate referrals and transitions to appropriate healthcare services. Requirements of the Travel Nurse Valid Tennessee Registered Nurse (RN) license.
