Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of clients by providing essential case planning and resources. You will help individuals gain independence and access necessary services while enjoying comprehensive benefits and a supportive work environment.
Key Responsibilities
Function as a case planner and supply appropriate interventions for client care
Develop action plans utilizing existing and new resources for client care
Conduct routine community contacts on behalf of social workers
Evaluate living conditions of clients through home visits
Provide guidance in independent living skills
Run groups under supervision of social worker
Perform individual/group counseling as needed
Conduct home visits to assess needs of family and child safety
Provide case management and access to services
Perform community outreach and establish linkages with helpful organizations
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
