Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams for care coordination
Provide education to patients and families
Maintain patient-centered documentation
Participate in administrative activities and clinical supervision
Support the development of hospital programming
Required Qualifications
Master's Degree in Social Work, Counseling, or Marriage and Family Therapy
LMSW, LPC-A, LMFT-A, or CADC
Previous experience in a healthcare setting
Valid Driver's License
Computer proficient
Excellent communication skills
Preferred Qualifications
Ability to assess problems and determine solutions
